Access Home Health is seeking a Registered Nurse (RN) for PRN positions in Daytona & Ormond Beach, FL. Build your schedule and provide compassionate, in-home, one-on-one care to patients with strong clinical support.

Qualifications

• Active Florida RN License is required.
• CPR Certification is required.
• Strong assessment skills are essential.
• Home health/OASIS experience is preferred but not required.

Responsibilities

• Conduct comprehensive assessments of patients’ health needs and develop individualized care plans.
• Administer medications and treatments as prescribed, monitoring for effectiveness and side effects.
• Provide education and support to patients and families regarding health management, disease prevention, and treatment options.
• Perform wound care, catheter care, and other nursing procedures as needed.
• Document patient progress accurately in electronic health records (Epic) and maintain regulatory compliance.
• Collaborate with physicians, therapists, and other healthcare professionals to coordinate care.
• Ensure infection control protocols are followed to maintain a safe patient environment.
• Participate in ongoing professional development and training opportunities.

Compensation Structure

• Salary range: $80,000 - $110,000/year
• Start of Care (SOC): 2.0 Units
• Evaluation / Re-Evaluation /Recertification / Resumption of Care: 1.5 Units
• Routine Visit: 1.0 Unit
• OASIS Discharge: $65
