Job summary

Emerald Home Health in Carnegie, PA seeks a Registered Nurse to provide and coordinate skilled nursing care for patients at home, following physician orders and agency policies.

You will perform assessments, develop and update care plans, supervise LPNs and aides, document care, and ensure compliance with Medicare/Medicaid rules; must have RN license, CPR, English communication, driver's license, and valid auto insurance.

Qualifications

• Graduate of an accredited school of nursing and current RN licensure.
• Current CPR certification and health certificate/TB testing.
• Valid driver's license and access to reliable automobile.

Responsibilities

• Performs initial and ongoing patient assessments in response to patients’ needs and in accordance with physician orders.
• Initiates a written plan of care, re-evaluates and updates the plan as necessary.
• Provides skilled nursing care within the nursing scope of practice and coordinates patient care and/or services.
• Documents skilled care and/or services provided in compliance with federal and state laws and agency policies.
• Supervises Licensed Practical Nurses, home health aides, homemakers and companions as required by state law.

Requirements & Qualifications

• Graduate of an accredited school of nursing
• Current state licensure as a Registered Nurse in the state.
• Experience as an RN commensurate with one of the following:
• One (1) year acute care experience within the last two (2) years.
• One (1) year home healthcare experience within the last two (2) years.
• One-year community health or public health experience within the last two (2) years.
• One (1) year outpatient facility experience within the last two (2) years
• Current CPR card
• Current health certificate/physical examination and TB testing results
• Valid state driver's license and reliable automobile
• Proof of current automobile insurance
• Able to communicate effectively in English, both verbally and in writing
