Burlington Health & Rehabilitation Center is seeking a dedicated Registered Nurse (RN) to join our team in Burlington, VT. The RN will provide high-quality patient care, collaborate with other healthcare professionals, and contribute to positive resident outcomes.

Responsibilities include supervising nursing assistants, administering medications, participating in admissions and transfers, and coordinating care plans.

Qualifications

Responsibilities

  • Direct the daily activities of certified nursing assistants in accordance with regulations and facility policies.
  • Ensure policies and procedures are followed by nursing personnel.
  • Participate in admissions, discharges, and transfers of residents.
  • Observe changes in residents' status and notify physicians and families; document accordingly.
  • Transcribe physician orders and carry out orders as written.
  • Prepare and administer medications per physicians' orders and monitor for adverse effects.
  • Perform wound treatments and document changes.
  • Schedule follow-up appointments and transportation as needed.
  • Provide nursing leadership to unit staff and supervise assigned personnel.
  • Provide direct care such as colostomy changes, tube feedings, wound care, suctioning, and IV administration.
  • Perform rounds to ensure resident needs are met and staff duties are performed.
  • Collaborate with interdisciplinary team to holistically meet residents' needs.
  • Report incidents or concerns and participate in investigations as needed.
  • Participate in end-of-life care as required.
  • Maintain adequate stock of medications and supplies and notify appropriate personnel.

Job Summary

Burlington Health & Rehabilitation Center is seeking a dedicated and compassionate Registered Nurse (RN) to join our team of healthcare professionals. As an RN at our facility, you will have the opportunity to provide high-quality patient care, work collaboratively with other healthcare professionals, and make a meaningful difference in the lives of our residents.

Responsibilities
  • Direct the daily activities of certified nursing assistants in accordance with current federal, state, and local regulations and guidelines, as well as established facility policies and procedures
  • Ensure policies and procedures are complied with by nursing personnel assigned
  • Participate in the admission, discharge, and transfers of residents as required
  • Observe for changes in residents' status, notifying the physician and resident's family or representative, and documenting accordingly
  • Transcribe physician orders to medical records and carry out orders as written
  • Prepare and administer medications as per physicians' orders and observe for adverse effects
  • Perform wound treatments as per physicians' orders, observe for changes, and document accordingly
  • Schedules follow-up appointments for residents and transportation needs as indicated
  • Provide nursing leadership to nursing personnel assigned to the unit
  • Provide direct care skills such as colostomy changes, tube feedings, wound care, suctioning, IV administration, etc. in accordance with current policies and procedures
  • Perform rounds to ensure resident needs are being met and personnel are performing their assigned duties
  • Collaborate with other members of the interdisciplinary team as needed to ensure residents' needs are holistically met
  • Inform appropriate departments of concerns or complaints and file grievances as necessary
  • Report any incidents or unusual occurrences to the supervisor, unit manager, assistant director, or nursing director and participate in the investigative process as needed
  • Participate in end-of-life care as required
  • Ensure there is adequate stock of medications, supplies, equipment, and notify appropriate personnel of needs
  • Initiate, review, and update care plans as required
  • Promote teamwork, mutual respect, and effective communication
  • Participate in the survey process when inspection surveys arise
Requirements
