Brockton Post Acute Care is hiring Registered Nurses to provide skilled clinical care in a 156-bed sub-acute and long-term care facility in Brockton, MA.

Qualifications

Responsibilities

• Provide resident care in accordance with nursing practice standards, physician plan of treatment, and written plan of care.
• Follow and implement care plans developed by the Nurse Manager.
• Perform medication administration and treatments per plan of care (POC).
• Monitor and report changes in residents’ conditions to the clinical team.
• Maintain accurate, timely documentation in line with physician orders, policies, and regulatory standards.
• Collaborate with interdisciplinary teams to ensure coordinated, high-quality care.
• Ensure compliance with infection control protocols and nursing regulations.

Responsibilities for Registered Nurse (RN):

• Provide resident care in accordance with acceptable standards of nursing practice and the physician’s plan of treatment, as well as the written plan of care.
• Follow and implement care plans developed by the Nurse Manager.
• Perform medication administration and treatments in accordance with the plan of care (POC).
• Monitor and assess residents’ conditions, promptly reporting changes to appropriate clinical team members.
• Maintain accurate, timely, and complete documentation in accordance with physician orders, facility policies, and regulatory standards.

• Ensure compliance with infection control protocols, safety standards, and nursing regulations.

Qualifications for Registered Nurse (RN):

• A current, unencumbered active license to practice as an RN in the state of MA.
• Valid BLS Certification (CPR) required.
• Ability to manage care for multiple residents simultaneously in a fast-paced healthcare environment.
• Strong clinical assessment, communication, and documentation skills.
