Job summary

Summa Home Health and Hospice LLC is seeking a Registered Nurse to provide comprehensive skilled care in patients' homes. You will educate patients and families, ensure patient safety, and document accurately within regulatory guidelines.

The role emphasizes OASIS assessments, care plan adjustments, and collaboration with physicians, coworkers, and care teams. A valid RN license with acute care experience is required; home health experience is preferred.

Qualifications

Responsibilities

  • Provide skilled nursing care in patients’ homes as ordered by the physician.
  • Observe and monitor patient conditions.
  • Perform OASIS assessments and adjust care plans as needed.
  • Administer medication per physician orders.
  • Reduce re‑hospitalizations by front‑loading visits for high‑risk patients.
  • Manage multidisciplinary care as applicable.
  • Promote continuity of care with appropriate admissions, transfers, and discharges.
  • Counsel patients and families on disease/injury processes and management.
  • Incorporate patients and families in care plan development.
  • Teach preventive measures to decrease re‑hospitalizations.
  • Listen to patients and families.
  • Oversee and supervise care provided by aides and LPNs.
  • Evaluate treatments and medical conditions per care plan.
  • Ensure level of care meets patient needs.
  • Communicate with LPNs and aides about supervisory visit results.
  • Complete all clinical documentation per protocol and federal guidelines.

The RN is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The RN is responsible for all practices and duties within the scope of practice as outlined by the state.

Essential Functions
  • Drive to patients’ primary locations per scheduled visits
  • Attend daily assigned visit locations
  • Document all aspects of subsequent, discharge, eval/recert/resumption visits within 24 hours
  • Document all aspects of start‑of‑care visits within 48 hours
  • Submit all required documentation within specified company requirements
  • Follow plan of care within scope of practice for an RN
  • Fulfill RN visits in assigned geographic location per patient needs within federal and state specifications
