Job summary

Elara Caring in Danbury, CT is seeking a Registered Nurse - Behavioral Health for day positions in the New Milford and Danbury areas. Enjoy flexibility and in-home care, avoiding traditional long facility hours.

As a New Milford-Danbury area day position, you’ll assess patients, manage caseloads, update care plans, educate families, and supervise staff. Must have RN license and 1+ year of clinical nursing experience; home health preferred.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate/Diploma/Associates/Baccalaureate).
• 1+ year of nursing experience in a clinical care setting.
• Home health experience preferred.
• Ability to lift up to 50 pounds and travel between patient homes.
• Valid driver’s license or reliable transportation.

Responsibilities

• Assess patients’ physical, emotional, social, and environmental needs.
• Manage caseload and develop/update individualized care plans.
• Complete initial assessments and ongoing evaluations during visits.
• Monitor patient progress and communicate with physicians and care team.
• Update care plans as needs change; educate patients, families, and caregivers.
• Teach behavioral health care and symptom management to staff in partner care settings.
• Supervise and support Behavioral Health Aides and LPNs.
• Report safety concerns to supervisors as needed.
• Provide compassionate end-of-life support and bereavement assessments.

As a New Milford Danbury Area Day Position, On Call, Holidays, You’ll Contribute To Our Success In The Following Ways

• Assess patients' physical, emotional, social, and environmental needs.
• Manage a caseload and develop, coordinate, and update individualized care plans.
• Complete initial assessments and provide ongoing evaluations during patient visits.
• Monitor patient progress and communicate changes with physicians, supervisors, and the care team.
• Update care plans based on patients' changing needs.
• Educate patients, families, and caregivers about the plan of care and behavioral health support.
• Teach behavioral health care and symptom management to staff in partner care settings.
• Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.
• Report safety concerns or other issues to supervisors as needed.
• Provide compassionate end-of-life support and complete bereavement assessments.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

Benefit eligibility can vary by position and shift status.

The salary range for this position is $82,000 to $101,000 annually, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.

This role may also be eligible for additional compensation, which could include performance-based bonuses, commissions, on-call pay, or other incentive earnings. These components are not guaranteed and depend on individual and/or team performance.
