Job summary

Ambercare Homecare in Taos, NM is seeking a Registered Nurse to join our team as a PRN clinician. You will provide home visits and coordinate care with the Clinical Services Manager to support patients in Taos and surrounding areas.

The role requires an active NM RN license, CPR certification, and at least one year of clinical experience, preferably in home health or related settings. Reliable transportation and a valid driver’s license are required.

Qualifications

• Registered Nurse licensed in New Mexico.
• Minimum 1 year clinical experience; home health preferred.
• CPR certification currently valid; own reliable transportation with driver’s license and auto insurance.

Responsibilities

• Provide visits as assigned by the Clinical Services Manager, including assessment and planning of care.
• Assess data from patient and family to coordinate intervention by the interdisciplinary team.
• Communicate with attending physician and other agencies for optimal care coordination.
• Provide visits to assess patient needs and update the plan of care.
• Perform nursing procedures as directed by physician.
• Assist in clinical pronouncement of death and family support as applicable.
• Maintain accurate patient records and care plans.

RN What You’ll Do (Responsibilities)

• Provide visits as assigned by the Clinical Services Manager, including the assessment, determination of eligibility, admissions processing, planning, implementation and evaluation phases of the nursing process. Assignments will vary depending on licensure.
• Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family’s needs and coordinate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide visits to assess the patient’s needs and update the plan of care
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to family, as applicable to licensure
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely

Qualifications

• Registered Nurse
• Graduate from an accredited registered nursing program and licensed in the state
• At least one (1) year clinical practical nursing experience preferably in intensive care, rehabilitation or medical surgical nursing (preferred)
• Home health experience preferred
• Possess and maintain valid CPR certification
• Must have reliable transportation, current driver’s license and appropriate automobile insurance
