Job summary

Legacy Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) for per diem shifts in the Fredericksburg area. You will provide hands-on hospice nursing, coordinate services, and guide patients and families through the hospice journey with empathy and clinical excellence.

The role emphasizes engaging with the Interdisciplinary Care Team, educating families, and participating in on-call duties to ensure timely, compassionate care. Excellent patient-centered support is offered.

Qualifications

• Hold a current unencumbered RN license.
• Minimum of 1 year RN experience in medical-surgical/acute care; hospice experience is a plus.
• Proficient computer skills with EMR experience.
• Current CPR certification if required by state.

• Empower Through Engagement: respond to clinical referrals with care and expertise.
• Collaborative Care Champion: work with Clinical Director to assess client eligibility for hospice services.
• Nurturing Direct Care: provide hands-on hospice nursing care and coordinate services.

• Educator and Advocate: educate patients and families and involve them in care plans.

Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Legacy Hospice is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Schedule: Per Diem, Monday - Friday, 8 am - 4:30 pm and On-Call or After Hours as Needed

Territory: Fredericksburg

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$38.00 - $43.00 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
