Job summary

Legacy Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our compassionate nursing team delivering end-of-life care throughout the community. You will coordinate services, support patients and families, and ensure care plans reflect comfort and dignity.

Schedule: Monday–Friday, 8 am–4:30 pm; Territory: Fishersville. You will collaborate with an interdisciplinary team to provide The Best Care Possible, grounded in clinical excellence, compassion, and connection.

Qualifications

• Hold current unencumbered RN license.
• At least 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient in EMR and computer skills.
• Valid driver's license and transportation.
• CPR certification as required by state.

Responsibilities

• Respond to referrals with care and expertise to ensure quality service.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Deliver direct hospice nursing care focusing on comfort and quality of life.
• Provide nursing care rooted in proven principles to ease pain.
• Educate and empower patients and families for personalized care plans.

Legacy Hospiceis seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

• Collaborative Care Champion: Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services. Your expertise will guide us in providing the right care, at the right time.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$80,000 - $90,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
