Job summary

ComForCare Boulder is seeking an experienced RN to join our home care team in Colorado. You will assess client status, create tailored plans, and deliver interventions focused on realistic outcomes.

This role requires current RN license and CPR, reliable transportation, and commitment to confidentiality and infection control. You'll collaborate with a multi-disciplinary health team to support clients and report progress.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current RN license.
• CPR certification.
• Access to reliable transportation.
• Maintains confidentiality in all aspects of the job.
• Knowledge of safety and infection control practices.

Responsibilities

• Perform comprehensive assessments of client status (physical, psychosocial, environmental).
• Develop individualized care plans using data analysis and current findings.
• Provide interventions focused on realistic client outcomes.
• Report changes to update the plan of care reflecting progress toward goals.
• Administer medications as needed.
• Maintain client records with assessments, planning, and evaluations.
• Initiate referrals to other health team members as needed.
