Job summary

ComForCare Home Care in New Jersey seeks a compassionate Registered Nurse (RN) to provide one-on-one private duty nursing in patients' homes. The RN delivers continuous skilled care, develops individualized plans, administers medications, and coordinates referrals per NJ Medicaid/MLTSS requirements and ACHC standards.

The role requires a BSN, NJ RN license, and at least two years in acute care with supervisory experience.

Qualifications

• Graduate of an accredited school of professional nursing; BSN required.
• Current New Jersey RN license in good standing.
• Minimum of two years nursing in an acute hospital setting or equivalent; supervisory experience.
• Excellent verbal and written communication skills.
• Must be a licensed driver with insured vehicle.
• Has CPR certification.

Responsibilities

• Provide direct, continuous skilled nursing care to adult patients in the home.
• Perform and document comprehensive nursing assessments and ongoing reassessments.
• Implement and follow an individualized Plan of Care (POC) approved by the physician.
• Administer medications, treatments, and therapies per physician orders.
• Monitor and manage complex medical conditions (e.g., tracheostomy, ventilator, G-tube, seizure management).
• Maintain accurate, timely clinical documentation in accordance with ACHC and NJ regulations.
• Educate and support patients and CHHAs regarding care plans and treatments.
• Communicate effectively with physicians, care managers, and interdisciplinary team members.
• Participate in case conferences, supervisory visits, and quality improvement activities.
• Ensure care is delivered in accordance with infection control and patient safety standards.

What we are looking for

• Graduate of an accredited school of professional nursing; BSN required.
• BSN in nursing and two years combined public health nursing and progressive professional responsibilities in public health nursing.
• Current license in good standing as a Registered Nurse in the state of New Jersey.
• Minimum of two (2) years nursing experience in an acute hospital setting or equivalent experience with one year of supervisory experience.
• Demonstrated ability to supervise and direct clinical and administrative personnel.
• Complies with accepted professional standards and practice.
• Excellent verbal and written communication skills and strong interpersonal skills.
• Knowledge of home care federal and state regulations.
• Knowledge of reimbursement sources and documentation requirements within home care.
• Must be a licensed driver with automobile insured in accordance with Agency and state requirements.
• Has current CPR certification.

What you will be doing

Hourly Range

• 32.00-40.00 per hour based on years of experience.
