Qualifikationen

• BSN required; ADN acceptable if VA contract allows.
• 2–3 years RN experience in primary or ambulatory care, government healthcare preferred.
• Must be credentialed and in good standing with VA.
• Unrestricted license in state of employment or as contract requires.
• Current BLS and ACLS certifications.

Aufgaben

• Assist practitioners with patient care, exams, and tests.
• Perform patient assessments and triage unscheduled visits.
• Serve as point person within PACT Team and coordinate with VA and non-VA providers.
• Provide patient and family education focused on self-management and prevention.
• Support enrollment, scheduling, and follow-up visits.
