Job summary

The Ridge Post-Acute, a 54-bed skilled nursing facility in San Jose, CA, seeks a Registered Nurse to deliver high-quality care to post-acute patients recovering from illness, surgery, or injury. You will assess conditions, develop care plans, and collaborate with physicians and staff to coordinate care.

This role emphasizes patient education, safety, and regulatory compliance. The ideal candidate holds an RN license in California with at least one year of post-acute or long-term care experience

Qualifications

• Current and valid RN license in the state of employment.
• Graduation from an accredited nursing program (ADN or BSN).
• BLS certification.
• Minimum of one year of clinical nursing experience, preferably in post-acute, rehabilitation, or long-term care settings.
• Strong knowledge of nursing principles, patient care standards, and medical terminology.

Responsibilities

• Conduct thorough patient assessments and document medical histories, symptoms, and vital signs accurately.
• Develop, implement, and update individualized nursing care plans based on patient needs and physician directives.
• Administer medications and treatments safely and monitor patients for adverse reactions or changes in condition.
• Collaborate with interdisciplinary teams to coordinate patient care and participate in care planning meetings.
• Educate patients and their families on post-acute care procedures, medication management, and health maintenance.
• Maintain compliance with healthcare regulations, facility policies, and quality standards.
• Respond promptly to patient emergencies and provide appropriate interventions.
• Supervise and mentor licensed practical nurses (LPNs) and nursing assistants as needed.
