Job summary

AHRC Nassau seeks a skilled nurse to provide medical oversight for Day Habilitation attendees and support staff. You will oversee medical assessments, ensure safety, and coordinate care with families, residences and physicians.

The role includes training staff, reviewing records, and maintaining compliant documentation. The position requires an Associate degree in Nursing, NYS Nursing license, and at least 6 months’ experience with people with disabilities.

Qualifications

• Associates degree in Nursing required.
• New York State Nursing license required.
• Six (6) months’ experience with people with disabilities required.

Responsibilities

• Provides medical oversight and medical assessment of all people attending Day Habilitation.
• Provide continuous training and support to the staff to meet medical needs.
• Administer first aid or guide staff on interventions as needed.
• Review medical records and follow doctor's orders.
• Collaborate with the interdisciplinary team to address medical concerns.
• Coordinate care with families, residences and MDs.
• Maintain comprehensive medical documentation per agency policies.
• Provide supervision to LPNs and assist with medical assessments as needed.
• Implement new medical policies and train staff.
• Ensure safety procedures are followed; report hazards immediately.
• Perform additional related duties as requested by supervisor.
• Physical tasks may include lifting and transferring.

Overview

To provide nursing oversight to people that attend the Day Habilitation program, and support and guidance to the staff.

Responsibilities

Primary duties and responsibilities include but not limited to:

• Provides medical oversight and medical assessment of all people that attend the Day Habilitation to ensure their safety and well- being.
• Provide continuous training and support to the staff in order for them to efficiently meet each person’s medical needs.
• Administer first aid as needed or instruct staff on necessary medical interventions.
• Reviews all medical records and paperwork, including annual physicals and medical visit appointments, on a regular basis and following doctor’s orders as prescribed.
• Collaborates with the interdisciplinary team to ensure all medical concerns are addressed.
• Acts as a liaison with the family, residences and MD to coordinate care.
• Ensure that all medical documentation is comprehensive and conforms with agency policies and procedures and is maintained in the person’s medical chart.
• Provide supervision to the LPN, and will be available to the LPN for any medical assessment or medical concern.
• Ensure implementation of all new medical policies and procedures by training staff and observation of practices.
• Ensure that work-related safety procedures are in place and are being adhered to by staff. Observance of an unsafe or a potentially hazardous work environment must be immediately reported to supervisor.
• Performs additional related duties as requested by her supervisor.
