Job summary

AHRC Nassau in Syosset, NY seeks an LPN to provide direct medical care for individuals in the Day Habilitation program, including gastrostomy feedings, suctioning, and medication administration under supervision. Overview

Performs tasks and responsibilities within the framework of health counseling and provision of supportive and restorative care under the direction of the RN or MD; assigned to attend to the medical needs of individuals in the Day Habilitation program; includes, but is not limited to direct administration of gastrostomy tube feedings (upon successful completion of OPWDD course), suctioning airways (under supervision of RN) medication administration, first aid and emergency response; provides supervision and medical oversight while accompanying individuals during transit to/from their homes and Day Habilitation program.

Provides supervision and medical oversight for individuals while on community opportunities; transports individuals to job opportunity sites or other community-based activities; assists individuals in achieving success and satisfaction in their community-based endeavors; completes all requested medical and program documentation (i.e. Daily Records; Program notes, etc. ) related to the care of the individuals

Responsibilities

Primary duties and responsibilities include but not limited to:

• Provides direct medical care for individuals in the Day Habilitation program, providing all essential medical services and treatments including, but not limited to gastrostomy tube feedings, suctioning airways (RN must be on-site); medication administration, etc..
• Accompanies medically frail individuals during transit as needed, between their homes and the Day Habilitation program
• Assists the RN by gathering medical data based on information provided by the family, physicians, residential provider, clinical and all other related documentation
• Provides feedback to the RN regarding completion of all required program and agency medical documentation
• Facilitates communication between family and program regarding the individual’s medical status
• Administers medication as prescribed, according to program and agency requirements
• Administers appropriate medical oversight and treatment, as directed by the RN
• Alerts RN and program administration to changes in the individuals (illness, injury) medical status, adhering to required program and agency requirements
• Completes medical and program/agency documentation as required
• Attends all required agencies and program training courses, which include but is not limited to professional training and seminars as requested by the Nursing Supervisor and/or administration
• Performs all other job-related duties as requested by supervisor.

This position requires regular and substantial unsupervised and unrestricted physical contact with people receiving services and, therefore, is subject to successful completion of fingerprinting/criminal background check.

Ensures that work-related safety procedures are in place and are being adhered to by staff. Observance of an unsafe act or a potentially hazardous work environment must be immediately reported to supervisor and/or safety committee.

Personal outcome measures training is required prior to completion of evaluations and assessments.

Qualifications

• Valid NY State Licensed Practical Nurse (LPN) license Required.
• Good oral and written communication skills required.
• AMAP/First Aid/CPR/SCIP-R Training preferred
• Valid NYS Driver License Required.
• You must Maintain a NYS Driver’s License and remain a qualified driver in accordance with the agency’s driver policy.
• Additionally, if you currently maintain an out-of-state driver’s license; you must transfer your license to NYS within 30 days of hire.
• Minimum one (1) year experience supporting people with intellectual and other developmental disabilities preferred
