Job summary

HQ Home Care-Certified in New York is seeking an RN to provide professional nursing care per the nursing process on a per diem basis, collaborating with the health care team.

Qualifications

• RN licensure required.
• BLS certification required.

Responsibilities

• Assess patients' physical, emotional and socio-economic needs.
• Identify problems using nursing process and physician plan of treatment.
• Develop a care plan with patient and family aligned to physician plan.
• Monitor and report changes in patient condition and response to treatments.
• Counsel patients and families on disease self-management, wellness, nutrition, safety, education and mental health.
• Perform screening and treatment procedures per plan and maintain competency.
• Explain test results and refer as appropriate; obtain lab specimens as needed.
• Participate in continuing education and QA/PI initiatives.
• Fulfill all compliance responsibilities related to the position.
• Perform other duties as assigned.

Summary:

Provides professional care to patients utilizing the nursing process in collaboration with other health care team members. Works on a per diem basis.

Responsibilities:

• 1. Provides assessment of physical, emotional socio-economic needs and environmental needs of patients and their families.
• 2. Identification of problems utilizing patient data base, nursing process and physician's plan of treatment.
• 3. Develops a plan of care in conjunction with the patient and family based on the patients' needs and consistent with the physician's plan of treatment.
• 4. Observes, evaluates and reports to the physician the patient's physical and emotional condition; reaction to drugs or treatments and changes in the patient's condition.
• 5. Assesses health status through observation, interview and analysis. Counsels patients and families in basic principles of disease self-management and wellness and applies information for related fields such as nutrition, safety, education and mental health.
• 6. Performs screening and treatment procedures according to the patient's plan of care and maintains competency levels to provide care.
• 7. Interprets findings of tests to individuals or families and makes appropriate referrals. Obtains laboratory specimens when indicated. Gives preventive immunization or treatments under medical supervision.
• 8. Attends required continuing education courses.
• 9. Participates in Quality Assurance and Performance Improvement Program.
• 10. Fulfills all compliance responsibilities related to the position.
• 11. Performs other duties as assigned.
