Job summary

Alpine Care Home Health in Austin, TX seeks a Registered Nurse Per Visit to provide intermittent skilled nursing services under an individualized Plan of Care. You will coordinate with other disciplines, supervise non-professional home health aides, and ensure continuity of care for patients in their homes.

The role requires active RN licensure, CPR certification, a valid driver’s license, and strong clinical judgment with excellent communication and documentation skills.

Qualifications

• Proficiency in clinical skills and in documenting patient care.
• Excellent verbal and written communication; proficient PC use.

Responsibilities

• Provide intermittent Skilled Nursing visits per patient Plan of Care.
• Notify agency of schedule changes as needed.
• Report significant findings to patient’s physician and Clinical Supervisor.
• Submit complete nursing notes, communication notes, and supervision notes per policy.
• Submit change orders per agency policy.
• Maintain open communication with the care team.
• Supervise Home Health Aide as per Medicare criteria.
• Attend staff meetings and in-services as required.
• Participate in PI data collection and PI team as requested.
• Follow agency policies and procedures.
• Assist with discharge planning per Medicare criteria and agency policy.
• Able to lift 40 pounds, perform repetitive movements, and drive 2–4 hours per day.
• OSHA Category 1 exposure/risk.
• Apply the 4Ms framework: What Matters, Medications, Mentation, Mobility.
• Other duties as assigned.

Job description

The Registered Nurse Per Visit provides intermittent skilled nursing services; communicates the patient's progress with other disciplines and directs, supervises and instructs non-professional home health aide staff in the provision of personal care to the patient.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Provides intermittent Skilled Nursing services including assessment, evaluation, procedures, teaching and training activities as outlined in the patient Plan of Care
• Provides Skilled Nursing visits according to visit schedule and notifies agency of need to alter schedule
• Reports significant findings to patient's physician and Clinical Supervisor as they occur
• Submits completed skilled nursing visit notes; communication notes and home health aide supervisory notes per agency policy on designated days as requested by Clinical Supervisor
• Submits change orders per agency policy
• Maintains open lines of communications to all members of the continuum of care team
• Supervises Home Health Aide and license and documents per Medicare criteria and per agency policy and procedure
• Attends staff meetings, team conferences and educational in-services per agency requirements
• Is responsible for obtaining information provided at skilled nurse in-services and meetings and demonstrates appropriate follow-up related to information given at meetings and in-services
• Participates in Process Improvement (PI) program by assisting with collection of data and serves on PI team upon request
• Follows agency policies and procedures
• Performs these and all other duties as assigned by the Administrator
• Participates in discharge planning process per Medicare Criteria and agency policy and procedure
• Able to lift 40 pounds from floor to shoulder
• Repetitive walking, standing, sitting, bending, and use of hands
• Able to drive a car 2-4 hours per day
• Exposure/risk category: OSHA Category 1
• Responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provides Age-Friendly Care
• Other duties as assigned

REQUIRED KNOWLEDGE, SKILLS AND EXPERIENCE

• Is currently licensed in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)
• Active CPR Certificate
• Must maintain a valid driver's license and good driving record
• The ability to make sound professional clinical judgment
• The ability to assess and document patient needs and formulate individualized patient care plans to meet those needs
• Proficiency in clinical skills
• Excellent verbal and written communication skills
• Proficiency in personal computer use including email, word processing, spreadsheet and clinical software
