Qualifications

• 1 year RN experience in a hospital or clinic required.
• Certifications: NYS RN license, BLS, ACLS, Infection Control Training; specialty certification upon hire preferred.

Responsibilities

• Demonstrate leadership in coordinating and delivering patient care.
• Coordinate an interdisciplinary plan of care.
• Ensure documented plan of care for all assigned patients.
• Ensure completion of patient care assignments.
• Delegate care tasks to team members within scope.
• Establish therapeutic relationships with patients and families.
• Mentor and orient associates during onboarding.
• Complete admissions assessments within 8 hours.
• Document patient and family teaching needs before discharge.
• Evaluate progress and adjust the care plan as needed.

What You'll Do

• Demonstrate leadership skills in the coordination and delivery of patient care.
• Coordinate an interdisciplinary plan of care.
• Assure 100% of assigned patients have a documented plan of care.
• Ensure completion of patient care assignment 100% of the time.
• Delegate aspects of care to team members consistent with their capabilities.
• Establish a therapeutic relationship with patients and families.
• Participate in the orientation and development of associates through mentorship.
• Complete 100% of admission assessments within 8 hours.
• Document teaching/learning needs of patients and families prior to discharge.
• Evaluate patient progress toward health goals and modify the plan of care accordingly.
