Job summary

Pathways Healthcare, a physician- and nurse practitioner-led home health and hospice organization, seeks an experienced nurse for our Medical Rehabilitation Program in Springfield, VA. The role focuses on patient-centered care at home, with a family-inclusive planning process and evidence-based nursing actions.

Responsibilities include assessing patient status, initiating and revising plans of care, administering treatments, documenting care, and collaborating with physicians.

Qualifications

• 2+ years’ nursing experience, preferably in geriatrics.
• Degree in Nursing from an accredited institution and an active Virginia license.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Valid driver’s license and own a reliable, insured vehicle.

Responsibilities

• Assess and evaluate patient status; write and initiate care plans; regularly re-evaluate needs.
• Develop a care plan with goals based on nursing diagnosis and involve patient and family.
• Administer medications and treatments as prescribed; document changes and outcomes.
• Communicate with the physician regarding patient needs and obtain orders as required.
• Provide a safe environment and participate in interdisciplinary group meetings.
• Participate in on-call duties; supervise ancillary personnel; ensure necessary items and services are available.

Our innovative Medical Rehabilitation Program offers a meaningful alternative to institutional rehab, helping people heal at home, with dignity and purpose.

Responsibilities:

• Assess and evaluate patient’s status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develop a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician in the physician’s plan of care.
• Complete, maintain and submit accurate and relevant clinical notes regarding patient’s condition and care given. Record pain/symptom management changes/outcomes as appropriate.
• Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition; obtain/receive physicians’ orders as required.
• Provide and maintain a safe environment for the patient.
• Attend interdisciplinary group meetings.

Additional Duties:

• Participate in on-call duties.
• Ensure that arrangements for equipment and other necessary items and services are available.
• Supervise ancillary personnel and delegate responsibilities when required.
• Assume responsibility for personal growth and development and maintain and upgrade professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfill the obligation of requested and/or accepted case assignments.
• Actively participate in quality assessment performance improvement teams and activities.
