Job summary

InterCommunity is seeking a Registered Nurse (RN) for the Detox unit to support triage, initial assessment, and medical care for up to 35 clients on a medically managed unit.

The RN will administer medications, monitor patient responses, document vitals, and collaborate with the healthcare team to refine treatment plans and ensure best patient outcomes.

Qualifications

  • Current/active State of Connecticut RN license, BLS and CPR certification.
  • EPIC experience preferred.

Responsibilities

  • Administer medications to patients on schedule and monitor for reactions.
  • Maintain accurate patient records and vital signs.
  • Record and report patient status changes to the healthcare team.
  • Coordinate with multi-disciplinary team to plan, implement, and evaluate care.
  • Direct or supervise less-experienced nursing staff as needed.
  • Refer patients to appropriate health resources and community services.
  • Attend required meetings and trainings as part of employment.

Summary:

The Registered Nurse (RN) Detox plays a critical role in executing InterCommunity mission to promote whole person heath. Under the supervision and direction of the Nursing Supervisor, the Registered Nurse is responsible for providing triage services which consists of an evaluation and assessment of medical and psychiatric status (based on admission criteria) and its relationship to the need of withdrawal management. The Registered Nurse also provides direct patient care to up to 35 clients on a medically managed unit.

Essential Duties & Responsibilities:

  • Timely administer medications to patients and monitor patients for reactions or side effects.
  • Maintain accurate, detailed reports and records.
  • Record patients' medical information and vital signs.
  • Monitor, record, and report symptoms or changes in patients' conditions.
  • Order, transcribe and reconcile medications per MD order as needed.
  • Facilitate effective crisis intervention.
  • Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans to include referral to higher level of care.
  • Modify patient treatment plans as indicated by patients' responses and conditions and collaborate within a multidisciplinary team to achieve best patient outcomes.
  • Direct or supervise less-skilled nursing or healthcare personnel.
  • Refer patients to specialized health resources or community agencies furnishing assistance.
  • Perform other duties as required.

*All agency staff must be awake during all scheduled hours.

*Per diem employees must accept at least one available shift within a 30-day period to maintain active employment status.

*All agency staff are required to attend all mandatory department/agency meetings and trainings.

*Attendance at required staff meetings and assigned trainings is now a condition of employment and will be treated as paid work time.

*All Support Staff are required to show proof of a CPR Certification within 60 days of official start date.

Requirements

Education &/Or Experience:

A current/active State of Connecticut RN license, Basic Life Support and CPR certification. EPIC experience preferred, but not required.
