Responsibilities
- Care for a specific patient population to achieve desired outcomes.
- Expand knowledge and skills, providing effective direct care as part of the interdisciplinary team to complex patients.
- Delegate, validate, evaluate, and coordinate care provided by other caregivers.
- Coordinate patient care/unit activities among nursing peers, physicians, and support services.
- Collect comprehensive data pertinent to the patient's health or the situation.
- Perform age and disease-specific assessments independently, focusing on physiologic, psychological, and cognitive status.
- Recognize clinical and diagnostic status changes.
- Identify variables in patient/family education needs based on age, disease, and cultural differences.
- Develop a plan of care with strategies and interventions to attain outcomes.
- Formulate patient/family-centered goals/outcomes based on assessment data.
- Coordinate a patient/family-focused plan of care that is prioritized and comprehensive.
- Communicate, coordinate, and implement the plan of care.
- Maintain ongoing communication with patients, families, and healthcare team members regarding patient status and care plans.
- Implement age and disease-specific care promptly and effectively.
- Provide or coordinate patient/family education and ensure understanding.
- Follow established protocols, communicate appropriately, and document events.
- Provide nursing care and treatments reflecting patient needs, advocate appropriately, and demonstrate evidence-based practice.
- Continuously document assessments, care plans, interventions, and evaluations electronically.
- Administer medications using advanced bar-scanning and documentation technologies for safety.
- Participate in multidisciplinary discharges to ensure safe transitions and continuum of care.
- Evaluate progress toward outcomes.
- Use critical thinking to achieve desired patient outcomes.
- Initiate interventions for changes in patient condition or unexpected responses to care.
Qualifications
- Minimum of an associate degree in Nursing or a diploma of Nursing.
- BSN in progress or willing to obtain within three years of hire.
- Six months or more of relevant nursing experience or active participation in Hackensack Meridian Health's Nurse Residency Program.
- Ability to learn quickly, adapt to changing needs, and demonstrate accountability, teamwork, ongoing education, and patient-focused care.
Preferred Qualifications
- National Certification in area of specialty (preferred when eligible).
Licenses and Certifications
- NJ State Professional Registered Nurse License.
- BLS/CPR certification by the American Heart Association within 60 days of hire.
