Base pay range

$34.00/hr - $50.00/hr

Purpose And Scope

Supports the mission, core values and customer service of Home Dialysis Services & Dialysis Care Center, LLC (HDS/DCC).

Adheres to the HDS/DCC Policies and Procedures and Compliance Program including all state and federal regulatory requirements.

The Dialysis Nurse- PD HHD is responsible for Providing and Coordinating exceptional care to peritoneal dialysis (PD) and home hemodialysis (HHD) patients including patient training, primary nursing, and patient education.

Primary Duties / Activities

  • Provides safe and effective training, direct patient care and delivery of home dialysis therapies to patients in compliance with standards outlined in the facility policy and procedure manuals, as well as regulations set forth by the state and federal agencies.
  • Provides oversight and supervision of all Patient Care Technicians, as assigned
  • Rotates with other licensed staff/Home Caregivers to ensure adequate patient care management coverage.
  • Participates in staff training and orientation of new staff as assigned.
  • Assists in the identification and evaluation and selection of candidates for the HDS Home Dialysis Program, including assessment of patients’ home environment.
  • Trains home patients and their families on the safe and effective operation and maintenance of all Home Program specific equipment through a formal Home Dialysis Training program.
  • Educates patients and family regarding End Stage Renal Disease (ESRD) and information specific to all Treatment Options available to patients with ESRD
  • Assesses patient and family readiness and ability to perform dialysis treatments at home and patients’ home environment for suitability in the ongoing delivery of home dialysis therapy.
  • Provides ongoing education to patients regarding their home dialysis or vascular access and home dialysis therapy, medications, and other related health conditions.
  • Provides safe and effective delivery of home dialysis treatment and support care to Home Dialysis Patients as well as Staff Assisted Home Hemodialysis patients.
  • Assesses patients’ response to home dialysis treatment by following predetermined protocols
  • Communicates patient related issues to the Clinic Manager or Clinical Area Manager and the patient’s physician as needed
  • Reports adverse patient events at home or in the facility/clinic as well as equipment and technology related problems to Clinic Manager or Clinical Area Manager and physician, including the correct documentation of such events.
  • Assists with the coordination of a required transfer of the assigned patient to the hemodialysis clinic when appropriate
  • Reviews, transcribes, and enters physician lab orders accurately into the Medical Information System as required.
  • Maintains appropriate skill level in the operation of all home dialysis therapy-related equipment, including the ability to troubleshoot equipment problems with the patient via telephone.
  • Participates in all required staff meetings.
  • Participates in care conferences & QAPI
  • Other duties, as assigned.

Customer Service

  • Responsible for supporting the values and customer service standards of HDS/DCC.
  • Accountable for outstanding customer service to all staff, clients and vendors
  • Develops and maintains effective relationships through effective and timely communication.
  • Initiates action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner.

Education/Qualifications

  • Graduate of an accredited School of Nursing, RN or BSN
  • Current state licensure as a RN
  • Current CPR card

Experience

  • Must be PC proficient and able to thrive in a fast-paced setting; experience in Microsoft Office Suite: Outlook, Word and Excel a must
  • Good presentation & communication skills
  • Must be productive & resourceful with the ability to meet deadlines

Other Qualifications

  • Dialysis nursing experience is preferred.
  • Must be a self-starter who can work independently yet function in a highly collaborative, fast-paced team environment.
  • Ability to multi-task, work under pressure and meet deadlines effectively
  • Excellent written and oral communication skills
  • Strong data entry, organizational and interpersonal skills
  • High integrity, team player with patient service focus and flexibility

Physical Demands And Working Conditions

Day to day work includes desk and personal computer work and interaction with prospective and current clients. The position requires travel between assigned facilities and various locations. Travel to scheduled staff meetings may be required.

The work environment is characteristic of a health care facility with air temperature control and moderate noise levels. The work environment is also characteristic of a dialysis center or a patient’s home and as such the employee may be exposed to infectious and contagious diseases/ materials.

Must be able to lift 30 pounds as needed.
