Responsibilities

• Assess patients' physical needs, pain, fall risk, medications, and discharge planning with appropriate interventions.
• Plan of Care is developed at admission using assessment data.
• Provide patient and family education.
• Evaluate patient responses to interventions.
• Demonstrate leadership in patient care.
• Assist in organizing Patient Care Activities.
