Job summary

Qualifications

Responsibilities

• Assesses patient health status and conducts comprehensive health assessments across body systems.
• Collaborates with physicians and health team to coordinate care and plan treatments.
• Provides wound care and skin care and manages pressure ulcers.
• Administers medications safely and documents accurately.
• Educates patients and families; supports discharge planning and follow-up care.
• Documents patient care activities and monitors outcomes for quality improvement.
