Job summary

Responsibilities

• Assess patient clinical and psychosocial needs and apply unit-specific standards.
• Develop, implement and evaluate a plan of care with patient and family.
• Coordinate with interdisciplinary team to ensure safe transitions and quality outcomes.
• Complete required clinical and quality documentation per RHS standards.
• Delegate and prioritize tasks to meet patient care needs and respond to emergencies.
