Qualifications

Responsibilities

• Promote the mission, vision, and values of Weston County Health Services (WCHS).
• Deliver skilled nursing care in the home, including assessment, planning, implementation, and evaluation of individualized care plans.
• Maintain accurate, complete patient records, documentation, and required reports.
• Administer medications as ordered and monitor for side effects or adverse reactions.
• Monitor, report, and document patient symptoms and changes in condition; take and record vital signs.
• Coordinate care with providers, family members, and other departments to ensure efficient, seamless services.
• Provide patient and family education, guidance, and support, including home care instructions.
• Order/coordinate diagnostic and clinical tests as required and communicate results appropriately.
• Support patient comfort and daily needs, including mobility assistance and hygiene support when needed.
• Adhere to accreditation, compliance, confidentiality, safety, and infection control standards.

For the RN role, you have the opportunity to earn a sign-on bonus of $3,000 with a 50% payment at hire and the remainder at 6 months.

What You’ll Do

What We’re Looking For

• Current Wyoming Registered Nurse (RN) license in good standing
• Graduate of an accredited nursing program (Associates or Bachelor's degree)
• Current BLS certification (or ability to obtain upon hire)
• Strong clinical assessment and care‑planning skills; able to recognize and respond to changes in patient condition
• Knowledge of medical terminology, medication administration, and safe patient care practices
• Excellent documentation and attention to detail; ability to maintain accurate patient records
• Strong communication and interpersonal skills; able to educate patients and collaborate with families and providers
• Ability to work independently, manage time effectively, and prioritize multiple responsibilities
• Professional judgment, integrity, and commitment to confidentiality and compliance standards
• Ability to handle challenging situations calmly and professionally
• Ability to meet physical requirements of the role, including occasional lifting/carrying up to 50 lbs. and travel within the service area
• Flexibility for occasional after‑hours calls and schedule variations as needed
