Job summary

Casa Esperanza is seeking a Massachusetts-licensed Registered Nurse to join our residential treatment program. You will perform nursing assessments, coordinate medical clearances, and manage medication administration as part of an integrated care team.

The role requires bilingual English/Spanish communication to support a diverse patient population. Responsibilities include documentation in the electronic health record, medication reconciliation with the Medical Director/NP, and patient

Qualifications

• Massachusetts Licensed Registered Nurse (RN) is required.
• Bilingual English/Spanish highly desirable given the patient population.
• Knowledge of EHR systems and patient confidentiality requirements.
• Knowledge of ASAM guidelines and Substance Use Disorder treatment.
• Familiarity with Medication Assisted Treatment for substance use disorders.
• Understanding of Mental Health Disorders and integrated care approaches.

Responsibilities

• Perform Nursing Assessments and CIWA/COWS assessments.
• Complete medical clearance exams at intake when needed.
• Administer medications, manage med passes and counts; document accordingly.
• Collaborate with the clinical team and Medical Director/NP on med reconciliation.
• Maintain up-to-date electronic health records and progress notes.
• Educate patients on diagnoses, medications, and safety.
• Ensure duty to warn and mandated reporting are fulfilled.
• Work within an integrated care team model with counselors and case managers.

About the role

Latinas y Niños is a licensed, residential treatment program thatoffers integrated substance use and mental health treatment in a residential setting. Our program provides patients with treatment, support, and a recovery-oriented peer community to help patients find their own path to recovery.

Latinas also offers treatment and services for pregnant and postpartum mothers. We have family rooms for mothers with children under ten and additional beds for single women. We help mothers reunify or arrange visitation with their children. Each patient works with a care team to create their personal care plan. Care plans include services that build life skills, promote recovery, and support independence. Our goal is to help patients find their path to successful, long-term recovery and thrive. Our services are trauma-informed and tailored to the unique needs of each patient.

As part of the Casa Esperanza, Inc. team you will provide services in a fully integrated, holistic, and collaborative manner using a team-treatment approach, to ensure the coordination of all client services, both across Casa programs as well as with external providers. This integrated model aims to facilitate clear communication amongst all providers and eliminate duplication of efforts or conflicting approaches to care; detail necessarily follow up and assignments; and ensure-wrap around support services are in place to maximize treatment effectiveness and clinical outcomes.

What you’ll do

• Complete Nursing Assessments, COWS, CIWA Assessments

• Complete medical clearance exam at time of intake when needed

• Complete Medication Pass and all required documentation

• Complete Medication Counts at start and finish of shift of all control and non-control substances

• Work closely with clinical team during MDT meetings to offer and provide client medical insight as part of case presentations.

• Consult with staff and attend case conferences, as needed

• Work collaboratively with Medical Director/Nurse Practitioner for medication reconciliation process.

• Work as part of an integrated care team model

• Directly work in collaboration with community providers for medication refills and reconciliation.

• Complete evaluations based on client request for medical attention, emergency and non-emergency triage and physical/mental health rounds for safety and condition monitoring

• Keep electronic health records up to date including progress notes and assessments of service.

• Educating clients on their current medical diagnosis, needs and medical histories; educating clients on medication information and usages

• Alerting Program Director when conditions worsen or if abnormalities arise.

• Work in collaboration with counselors/case managers/medicaction specialists to ensure proper medication management and medication planning

• Work directly with Program Director on any medication errors and medication monitoring procedure

• Work uniformly with Recovery Specialist on medication monitoring procedure

• Ensure responsibilities regarding Duty to Warn and Mandated Reporting are fulfilled

Nursing Services

• Obtain vital signs and perform clinical screenings.

• Conduct medication reconciliation.

• Administer injectable medications as ordered.

• Conduct CLIA-waived point of care testing as needed

• Provide wound care and basic nursing interventions within the scope of practice.

Qualifications

Massachusetts Licensed Registered Nurse

Bilingual English/Spanish highly desirable due to population served

KNOWLEDGE/SKILLS/ABILITIES:

• Knowledge of EHR systems

• Ensure patient confidentiality in accordance with all federal, state, and local laws and regulations

• Knowledge on best practices associated from the American Society of Addiction Medicine (ASAM)

• Knowledge of Substance Use Disorder

• Knowledge of Medication Assisted Treatment for substance use disorders

• Knowledge of Mental Health Disorders

Schedule

• Tuesday and Wednesday; 7am - 3pm
