Job Overview

The registered nurse assumes accountability for providing individualized, age-specific care to patients from admission to discharge. Responsibilities include direct patient care, coordination with healthcare team, patient education, and discharge planning, adhering to organizational standards and regulatory requirements.

Responsibilities

• Assess patient status upon admission and document health data accurately.
• Coordinate interdisciplinary care and perform joint home visits with the Charge Nurse.
• Develop, implement, and update individualized care plans with patient and family involvement.
• Provide education and support to patients and families throughout care.
• Communicate effectively with the healthcare team and document all care activities.
• Make appropriate referrals and participate in multidisciplinary meetings.

Qualifications

Required:

• Bachelor of Science in Nursing (BSN) or Associate Degree Nursing (ADN) with commitment to obtain BSN within three years.
• Current Illinois RN license.
• American Heart Association BLS certification.
• Strong interpersonal, collaboration, and analytical skills.
• Valid driver’s license and auto insurance.

Preferred: Clinical experience in relevant area.
