Job summary

Mercy Home Health is seeking a Full-time Registered Nurse for its Palliative Care program based in Bucks County, PA. You will deliver skilled nursing care at patients' homes, develop care plans, administer medications and educate families, with a focus on top-of-license practice and autonomy.

Qualifications

• BSN degree required and eligible for PA RN license.
• Current PA RN license and CPR certification.
• Hospice/Palliative Care Certification required.
• Home health experience preferred.
• Strong interpersonal, organizational, problem‑ solving, critical thinking and priority‑setting skills.
• Ability to work independently and as part of a team.
• Proficient in computer skills.

Responsibilities

• Deliver skilled nursing care in patients' homes and create individualized care plans.
• Administer medications and monitor patient status and reactions.
• Educate patients and families on care plans, medications and safety.
• Support recovery and independence with compassionate care.
• Practice at the top of your license with autonomy and flexibility.
• Build meaningful 1:1 patient relationships and ensure quality documentation.
