PACT is an intensive and highly integrated approach for community mental health service delivery. The multidisciplinary team shares a caseload of individuals who experience symptoms of the most serious forms of mental illness, predominantly but not exclusively the schizophrenia spectrum disorders. Many individuals also experience substance abuse, victimization and trauma, psychiatric hospitalizations, arrests and incarceration, homelessness, and additional significant challenges. The symptoms and complications of mental illnesses lead to serious functioning difficulties in several areas of life, often including work, social relationships, residential independence, money management, and physical health and wellness.

The PACT Team participates in rotating weekend and holiday shifts to provide coverage for our clients during non-business days. If a weekend day or holiday shift is worked by a PACT employee, the PACT employee will be able to take a different day off that same week.

In collaboration with a psychiatric care provider, the team nurses assist with medication management and administration, through documentation as well as monitoring clients for medical problems or side effects. Nurses will communicate and coordinate services with other medical providers, engage in health promotion, prevention, and educational activities. The nurse will educate other team members to help them monitor psychiatric symptoms and medication side effects and work with clients to develop strategies to maximize the taking of medications as prescribed.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

  • Provide symptom and medication assessment, evaluation, education, and management to individuals with persistent mental illness. This includes consultation with Psychiatrist, MHCP, hospital, community providers, client, and client’s family/guardian for care.
  • Process and sign medication orders, complete medication refills and prior authorization for specific medications based on provider request.
  • Administer medications to consumers as directed by FBH Psychiatrist.
  • Ensure access to medications through timely refills, by obtaining sample medication per order and by appropriate use of FBH discretionary funds or patient assistance programs for medications.
  • Ensure pertinent medication-related information is conveyed to hospital staff in the event of client hospitalization.
  • Participate in rotating weekend and holiday shifts as needed.
  • Assist in development and delivery of staff and/or consumer education in the form of group classes or training.
  • Identify and include the client’s support network and applicable involved community partners in care coordination.
  • Develop and implement care plans, measurable objectives, and obtainable goals, in collaboration with the client, evaluating client progress and adjusting as needed.
  • Assist clients in identifying and accessing resources and collaborate with community resources on behalf of clients.
  • Assist clients in the development of healthy coping mechanisms in individual or group settings.
  • Maintain client records and all other required documentation in a timely and confidential manner in accordance with agency policies and procedures.
  • Conduct risk assessments, develop and implement safety plans, and provide crisis intervention and stabilization services as needed, including facilitating access to inpatient facilities for behavioral health care.
  • Represent the agency and its services to the community in a positive and professional manner.
  • Participate in required staff meetings, including regular supervision and daily clinical staff meetings.
  • Participate in any required training, consultation calls, and onsite visits or review when required by contract.
  • Accept delegation and follow through with specific expanded assignments and responsibilities within FBH system such as quality improvement teams, task groups, committees, consultations, mentoring, or training responsibilities.
  • Establish and maintain collaborative working relationships with clinical, administrative, support staff, community partners, clients, and family/natural supports.
  • Demonstrate teamwork and goal-oriented productivity.
  • Perform FBH charting requirements, correspondence, Workday and other paperwork in a complete and timely manner.
  • Operate as a member of a multi-disciplinary team and ensure services are provided in the community and clinical settings as required by contract.
  • Other duties as assigned.

QUALIFIED CANDIDATES WILL HAVE:

REQUIRED:

  • RN licensed in the State of Washington.
  • Active BLS within 90 days of hire and must maintain active BLS.

PREFERRED:

  • One year of experience working with adults with severe mental illness.

KNOWLEDGE, SKILLS & ABILITIES:

  • Must have reliable transportation, insurance, and an active driver’s license and be willing to use a personal vehicle in the course of work.
  • Knowledge of Washington Administrative Codes and statutes pertaining to the treatment and delivery of mental health services, or ability to obtain within 3 months of employment.
  • Knowledge of service delivery models including groups, skills training, brief/focused therapy, community-based interventions for adults with serious and persistent mental illness, or ability to obtain within 3 months of employment.
  • Ability to communicate and engage effectively with individuals of varied backgrounds, trauma experiences, cultures, education levels, and socio-economic status.
  • Must possess excellent communication skills, including the ability to send and receive oral and written messages, in a clear, positive, prompt, and appropriate manner, while maintaining customer confidentiality.

  • This position provides client services in community settings on a daily basis. Community settings include but are not limited to residences, group/congregate living settings, hospitals, shelters, the streets, or anywhere in the community at large. Some environments may not be ideal in terms of cleanliness or being habitable.
  • Daily travel between locations is required.
  • The ability to drive a vehicle, traverse distances up to several city blocks and on various terrains, navigate stairs, get in and out of vehicles frequently is required. Daily use of a computer is required.
  • May be required to lift 10 - 20 pounds.

*The following terms and conditions apply to new $2,000 Sign On Bonus:

  • Internal transfers into PACT are not eligible for the sign-on bonus, except for PRN employees who accept a full-time PACT position.
  • If you receive the $2,000 sign-on bonus, you will not be eligible for an internal transfer out of PACT for one year, absent extenuating circumstances and approval by the Chief Human Resources Officer.

PAY RANGE (Salary/Exempt): $82,386.88 - $100,370.85*

*All pay is based on longevity at Frontier Behavioral Health; thus, the starting pay is $82,386.88 annually.
