Key Responsibilities:

• Conduct comprehensive nursing assessments and evaluations


• Develop, implement, and monitor individualized care plans


• Ensure accurate, timely clinical documentation


• Coordinate with interdisciplinary teams and attend treatment meetings


• Monitor medical needs, appointments, and follow-ups


• Supervise AMAP staff and ensure compliance with medication protocols


• Maintain compliance with OPWDD regulations and standards


• Provide on-site clinical support and guidance to staff


• Participate in case conferences, trainings, and committee meetings


• Serve as liaison between residence and medical providers

Requirements:

• Active New York State RN license


• Experience working in an OPWDD or similar setting required


• Strong clinical assessment and documentation skills


• Excellent communication and organizational abilities


• Ability to work independently and handle multiple priorities
