Registered Nurse - Nurse Navigator REMOTE

Our expanding system includes locations across Southern California, Orange County, and centers in Atlanta, Chicago, and Phoenix.
The Nurse Navigator RN is a vital member of the multidisciplinary team, acting as an advocate and educator for patients from initial intake through diagnosis and treatment planning. Responsibilities include assessing patient needs, coordinating care, and serving as a liaison among patients, families, and healthcare providers, while complying with hospital policies and state Nurse Practice Acts.
Work Monday - Friday, 8:00 am - 6:00 pm PST.
Assess cancer treatment history, evaluate patient suitability for treatment at City of Hope, and coordinate pre-admission evaluations and documentation.
Promote collaboration within and across departments, foster positive relationships, and seek continuous service improvement and professional development.
Facilitate patient admissions, develop care plans with multidisciplinary teams, and ensure patient and family education on treatment plans.
Support departmental goals, participate in performance improvement, and ensure timely documentation and workflow management.
Apply problem-solving techniques during intake, coordinate travel needs, and ensure comprehensive patient records.
Maintain knowledge of patient safety protocols, hospital variance reporting, disaster procedures, and organizational policies.
Graduate of an accredited nursing school.
Valid RN license in California, to be verified within 90 days of employment.
BLS certification.
Oncology Certified Nurse (preferred).
