Registered Nurse - Nurse Navigator REMOTE

Job Responsibilities

The Nurse Navigator RN acts as a member of the multidisciplinary team, advocating and educating patients from intake through diagnosis and treatment planning. Responsibilities include:

1. Assessing patient history, including diagnosis and treatments, and evaluating clinical appropriateness for travel and treatment.
2. Serving as a liaison among patients, families, primary care providers, specialists, and care teams.
3. Ensuring compliance with hospital policies and state Nurse Practice Acts.
4. Documenting in EMR and coordinating outside records and imaging.
5. Supporting interdepartmental collaboration and patient education.
6. Facilitating admissions, developing care plans, and addressing patient concerns.
7. Participating in quality improvement and workflow planning.
8. Applying problem-solving techniques during intake and evaluating travel needs.
9. Adhering to patient safety protocols and organizational policies.

Qualifications

• Graduate of an accredited nursing program
• Valid California RN license, with verification within 90 days of employment
• BLS certification
• Oncology Certified Nurse (preferred)
