Job summary

Hospice of the North Coast seeks an experienced RN to provide skilled and palliative care under the hospice care plan, coordinating with the physician and the interdisciplinary team to support patients and families.

Graduate of an accredited school of nursing with a CA RN license (BSN preferred) and at least one year of recent nursing experience are required; a valid driver’s license and auto insurance are also needed.

Qualifications

• Current CA RN License; BSN preferred.
• Certification in Hospice and Palliative Nursing desired.
• Minimum of one-year nursing experience in the last 3 years or a NLN-accredited BSN.

Responsibilities

• Follow the hospice care plan and provide skilled and palliative care.
• Coordinate services with the attending physician and the interdisciplinary Hospice team.
• Participate in patient care conferences and supervise Home Health Aides as assigned.
• Provide education and supportive care to patients and caregivers.

NOC-12:00am-8:30am

$2.00 shift differential for night shift

POSITION SUMMARY:

The RN is an experienced registered professional nurse who follows the hospice care plan, provides skilled and palliative care and coordinates the services provided to his/her patients. The RN works cooperatively with the attending physician, and other members of the interdisciplinary Hospice team. The RN participates in patient care conferences and supervises the Home Health Aides assigned. The RN provides care, as described in this position description, to patients who have been diagnosed with a terminal illness. The RN regularly reviews and follows the plan of care. Performs prescribed medical treatments, including pain management and symptom control, conducts assessments and evaluations, reports all changes to the RN case manager, provides education and supportive care to patient and caregiver.

QUALIFICATIONS:

Education:

Graduate of an accredited school of nursing

Credentials:

Current CA RN License. BSN preferred. Certification in Hospice And Palliative Nursing desired

Experience:

Minimum of one-year experience as a professional nurse within the last 3 years or have a Baccalaureate in Nursing from a program accredited by the National League for Nursing

Core Competencies:

Demonstrated competency in performance of professional nursing functions. Requires excellent oral and written communication skills. Ability to work with dying patients and their families. Possesses a sympathetic attitude toward caring for the hospice patient and demonstrates positive communication skills in interacting with other members of the hospice team
