Job summary

The Center at CenterPlace in Greeley, CO seeks a team-oriented Registered Nurse (RN) dedicated to patient outcomes and customer service. This short-term rehabilitation facility features 54 private suites and state-of-the-art rehabilitation equipment, delivering care in a hotel-like setting.

Essential duties include administering meds, assessing patients, communicating condition changes, educating patients, ensuring care plans are followed, and supervising LPNs and CNAs.

Responsibilities

• Administers and documents medications and treatments per physician orders.
• Assesses patients routinely and with changes of condition.
• Communicates changes of condition, test results, etc. as per company policy.
• Educates patients as necessary.
• Ensures the patient care plan is implemented.
• Supervises and coordinates LPNs & CNAs.
