Job summary

UChicago Medicine AdventHealth in Woodridge, IL seeks a registered nurse for full-time day shifts. The role involves assessing patient needs, planning and coordinating care with the team, and delivering education to patients and families. May travel to Hinsdale clinic; schedule: Monday–Friday with potential 10-hour shifts.

Requires IL RN license, BLS, and nursing degree; experience in critical care and medical office is preferred. Benefits start day one and include strong well-being resources.

Qualifications

• Active RN license in Illinois (Required)
• BLS Certification (Required)
• Associate's Degree in Nursing (Required)
• Bachelor's Degree in Nursing preferred
• 1+ year medical office experience preferred
• 5+ years critical care experience preferred
• Proficient in documentation and patient education

Responsibilities

Job Description:

Location: 2363 63rd Street, Woodridge, IL (may travel to clinic in Hinsdale)

Schedule: Monday-Friday 8:30a-5p (possibility of 10-hour shifts)

• Assesses patients' physical, psychosocial, cultural, and spiritual needs through observation, interviews, and review of medical records and clinical data.
• Develops and implements patient care plans in collaboration with the interdisciplinary team, observing outcomes in accordance with nursing standards.
• Demonstrates competency in therapeutic and diagnostic interventions, including IVs, wound care, medication administration, catheterization, and specimen collection.
• Advocates for and protects the health, safety, and rights of each patient.
• Provides patient and family education based on assessment of learning readiness, needs, and ability, and evaluate the effectiveness of teaching.
• Encourages patient engagement in healthcare through motivational interviewing, self-management protocols, and teach-back methodology.
• Delivers relationship-based care, engaging patients in all aspects of care with respect for cultural differences and diversity.
• Delegates tasks to non-RN personnel in accordance with State Board of Nursing and hospital policy.
• Other duties as assigned. Coordinates and implements group visits for identified high-risk patient populations.
• Aids in the care transition of high-risk patients, including medication reconciliation and coordination of services.
• Contributes to continuous process improvement by actively participating in patient experience and satisfaction initiatives.
