Job summary

UChicago Medicine AdventHealth is hiring an RN to assess and plan holistic patient care at our Woodridge, IL site. You’ll collaborate with an interdisciplinary team to deliver safe, evidenced-based care and teach patients and families.

Responsibilities include implementing care plans, performing clinical tasks (IVs, wound care, meds), and supporting high-risk patients through care transitions in a fast-paced, compassionate environment.

Qualifications

• Associate's degree in nursing required.
• Bachelor's degree in nursing preferred.
• Registered Nurse (RN) license required.
• Basic Life Support (BLS) certification required.
• 1+ years of medical office experience preferred.
• 5+ years of critical care experience preferred.

Responsibilities

• Assess patients' physical, psychosocial, cultural, and spiritual needs.
• Develop and implement patient care plans with the interdisciplinary team.
• Demonstrate competency in therapeutic and diagnostic interventions (IVs, wound care, meds).
• Advocate for health, safety, and rights of each patient.
• Educate patients and families; assess learning needs and outcomes.
• Encourage patient engagement through motivational interviewing and teach-back.
• Deliver relationship-based care with cultural sensitivity.
• Delegate tasks to non-RN staff per policy; coordinate group visits.
• Support high-risk patients through transitions and coordinated care.
• Contribute to continuous process improvement via patient experience initiatives.

Job Description:

Location: 2363 63rd Street, Woodridge, IL (may travel to clinic in Hinsdale)

Schedule: Monday-Friday 8:30a-5p (possibility of 10-hour shifts)

• Assesses patients' physical, psychosocial, cultural, and spiritual needs through observation, interviews, and review of medical records and clinical data.
• Develops and implements patient care plans in collaboration with the interdisciplinary team, observing outcomes in accordance with nursing standards.
• Demonstrates competency in therapeutic and diagnostic interventions, including IVs, wound care, medication administration, catheterization, and specimen collection.
• Advocates for and protects the health, safety, and rights of each patient.
• Provides patient and family education based on assessment of learning readiness, needs, and ability, and evaluate the effectiveness of teaching.
• Encourages patient engagement in healthcare through motivational interviewing, self-management protocols, and teach-back methodology.
• Delivers relationship-based care, engaging patients in all aspects of care with respect for cultural differences and diversity.
• Delegates tasks to non-RN personnel in accordance with State Board of Nursing and hospital policy.
• Other duties as assigned. Coordinates and implements group visits for identified high-risk patient populations.
• Aids in the care transition of high-risk patients, including medication reconciliation and coordination of services.
• Contributes to continuous process improvement by actively participating in patient experience and satisfaction initiatives.
