Job summary

LiveWell Homecare Agency in Danville, PA seeks a registered nurse to educate patients, coordinate pre- and post-hospital care, and act as a liaison between patients and healthcare teams.

You will support care plans, work with multidisciplinary providers, and guide patients through care processes while maintaining follow-up and ensuring policy compliance.

Qualifications

• Graduate from a Specialty Training Program in Nursing (required).
• Bachelor’s Degree in Nursing preferred.
• At least 2 years of RN experience required.
• Strong patient communication, coordination, and clinical skills.
• Ability to work in a clinical environment and collaborate effectively.
