Live Well Homecare in Danville, PA is seeking an experienced registered nurse to serve as a patient navigator, coordinating pre- and post-hospital care and facilitating entry into care programs.

You will educate patients and families, support multidisciplinary teams, and ensure accurate clinical documentation while promoting patient-centered care.

Competenze

• Graduate from a specialty nursing training program (required).
• Bachelor's degree in nursing preferred.
• Minimum of two years RN experience.
• Strong communication and teamwork skills.
