Job summary

PruittHealth is hiring a Registered Nurse Navigator to support patients and families through care planning and transitions. The RN will coordinate with physicians, therapists, social workers and other teammates to develop and implement individualized care plans, while upholding clinical standards.

Qualifications

• Active, current, unrestricted Registered Nurse (RN) licensure in the state of practice.

Responsibilities

• Provide compassionate, patient-centered nursing care and coordination.
