Job summary

All Heart Home Care in Nashville seeks an experienced Registered Nurse to join our home health team. This full-time role delivers compassionate, patient-centered care in patients' homes, collaborating with a dedicated care team to achieve optimal outcomes.

The RN will perform in-home assessments, develop care plans, administer medications, educate families, and coordinate with physicians. Travel within service area is required; TN RN license and CPR are required; home health experience preferred.

Qualifications

• Current RN license in Tennessee.
• Current CPR certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work independently and manage a patient caseload effectively.
• Home health experience preferred but not required.

Responsibilities

• Perform in-home patient assessments and develop individualized plans of care
