ACG Hospice is seeking a Registered Nurse to join our compassionate team. You will evaluate patients, develop care plans, and coordinate with families and the care team to ensure quality, patient-centered care.

Qualifications

• Must be a graduate of an accredited nursing program with a current RN license.
• Minimum of 2+ years nursing experience in a clinical setting (hospice preferred).
• Reliable transportation and physical ability to lift up to 100 lbs.

Responsibilities

• Evaluate patients and create care plans in collaboration with the care team.
• Communicate with patients, families, and healthcare professionals to coordinate care.
• Serve as the driver of the care team to ensure high-quality patient experiences.
