Job summary

WorkFit Medical Staffing, PLLC is seeking an LPN for long-term care with a pay rate of $40.00+ per hour. The role involves day/evening rotating shifts, 5x8 hours, every other weekend and holiday, on a 26-week contract.

You will perform assessments, implement care plans, administer medications, monitor residents, and collaborate with a multidisciplinary team. The position requires NY State RN license (note: RN license listed), BLS certification, and prior long-term care experience.

Responsibilities

• Monitor vital signs, symptoms, laboratory results, and overall health status.
• Respond promptly to emergencies and changes in residents’ conditions.
• Communicate significant findings and changes in condition to physicians and other healthcare providers.
• Supervise and collaborate with LPNs/LVNs, CNAs, and other care staff.
• Provide direction and support to nursing staff while maintaining appropriate delegation and supervision.
• Coordinate admissions, transfers, discharges, and follow-up care.
• Maintain accurate and timely clinical documentation in accordance with facility policies and regulatory requirements.
• Educate residents and families regarding medications, treatments, disease management, and ongoing care.
