Job summary

Community House Mental Health Agency in Seattle is seeking an RN/LPN to join a 24-hour residential care team. You will perform basic client evaluations, administer medications, and coordinate with prescribers to ensure continuity of care.

Qualifications

• Active WA State RN or LPN license.
• Current First Aid/CPR certification or ability to obtain within 90 days of hire.
• Experience working with people with severe and persistent mental illness.
• Must hold a valid driver’s license with clean driving record to use agency vehicles.
• Have legible handwriting.
• Ability to do simple mathematics.
• Basic computer skills.
• Ability to read and comprehend agency/member charts.

Responsibilities

• Perform basic physical evaluation of clients, including blood glucose monitoring and vital signs.
• Provide medication services for clients including MARS chart reviews, injections, and blood draws as needed.
• Evaluate treatment effectiveness with medical/clinical team and revise documentation as needed.
• Provide individual treatment sessions for emergencies and per prescribed treatment plan.
• Assist in discharge planning with written summaries for referrals and releases of information.
• Coordinate with agency prescribers to ensure refills and communications with pharmacies.
• Provide immediate crisis intervention and resolution as needed.
• Visually supervise medication distribution as necessary.
• Provide consultation, education, and training to staff and contract employees.
• Maintain client records in keeping with guidelines and confidentiality.
• Attend scheduled meetings to maintain RN/LPN licensing.
• Other duties as assigned.

CHMHA has a fantastic opening for a Registered Nurse/Licensed Practical Nurse (RN/LPN).

Our RN/LPN will work as part of a team to provide mental health services in 24-hour residential care facilities. They will coordinate with staff prescribers and outside primary care providers as needed.

SCHEDULE

The schedule for this shift is Monday-Friday, 35 hours per week.

RESPONSIBILITIES

• Perform basic physical evaluation of clients, which includes blood glucose monitoring, blood pressures, and other basic health screenings
• Provide medication services for clients who require such services including client MARS chart reviews, injections, and blood draws as needed.
• Evaluate treatment effectiveness with agency medical/clinical team for voluntary and involuntary clients. Revise treatment documentation as needed
• Provide individual treatment sessions when appropriate to emergency needs and per prescribed treatment plan.
• Assist in discharge planning by providing written summaries of treatment as needed for member referrals and releases of information.
• Coordinate with agency prescribers to ensure refills and other communications with pharmacies.
• Provide immediate crisis intervention and resolution as needed.
• Visually supervise medication distribution as necessary
• Provide consultation, education, and training to the staff at Community House and assigned contract employees as needed
• Maintain client records in keeping with agency, county, and state guidelines
• Maintain member and staff confidentiality at all times.
• Attend scheduled meetings and conferences as assigned and in meeting requirements for RN/LPN licensing
• Other duties as assigned
