Job summary

Signature HealthCARE in Clarksville seeks an experienced Registered Nurse to join our compassionate team at a 120-bed facility. You will provide direct nursing care, supervise nursing staff, and ensure high-quality resident outcomes.

You will work within an interdisciplinary team, maintain thorough documentation, manage admissions/transfers, and participate in QA activities. A current RN license in Tennessee, and at least 1 year of clinical nursing experience are required.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of Federal and State long-term care regulations.
• Strong verbal and written communication skills.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies.
• Monitor and observe residents’ physical and emotional status; notify attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
