Qualifications

• Education: Nursing Diploma/Associate's degree required.
• RN licensure required.
• Preferred: BSN degree.
• Med/Surg experience preferred.

Responsibilities

• Provides direct patient care activities including assessment, diagnosis, planning, implementation and evaluation.
• Educates patients and families on illness management, home care needs and medications.
• Promotes patient safety, quality and regulatory compliance.
• Participates in professional development and plans of care with the interprofessional team.
• Communicates the plan of care across the continuum of care.
