Registered Nurse, ICU experience: 2 years. Licenses: RN (Compact Accepted). Certifications: ACLS, BLS, CERT-NIH Stroke Scale. SSN and DOB required. Driver’s License copy on file. UHS specific employment history cover page to be completed with recruiter. Two professional references within the last two years. Flu vaccination is required; declinations are not accepted.
