Job summary

Trios Health in Kennewick, WA is seeking a Registered Nurse Intake/Assessment Coordinator (PRN) to conduct clinical assessments, coordinate admissions, and serve as a liaison between patients, families, referral sources, and care teams. This role helps ensure timely access to behavioral health services and a positive patient experience.

You will verify insurance, manage documentation, advocate for patients, and educate partners about admission criteria.

Qualifications

• Education: Associate's Degree Nursing (ADN) Required; Bachelor's Degree Nursing (BSN) Preferred.
• Current RN license in the state of practice.
• BLS and Handle With Care (HWC) certification within 45 days of hire.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Complete patient assessments and referral screenings.
• Gather clinical and demographic information and determine admission appropriateness.
• Coordinate admissions, including insurance verification, authorizations, physician orders, and required documentation.
• Communicate with patients, families, providers, and referral sources throughout the admission process.
• Maintain referral tracking and support census and bed management efforts.
• Serve as a patient advocate and provide ongoing support to patients and families.
• Build relationships with community partners and educate referral sources on admission criteria and services.
