Job summary

Just-In Home Care in Maryland seeks an RN Care Manager to design person-centered plans of care, mentor caregivers, and champion quality that keeps seniors safe, healthy, and comfortable at home.

You'll assess clients in their homes, craft care plans with families, coach staff, document in a HIPAA-secure EMR, and collaborate with physicians and therapists to keep care seamless and client-centered.

Qualifications

• Active RN license in MD.
• Bachelor’s in Nursing or related field – OR – 2+ years’ geriatric/home-care experience.
• Solid clinical assessment skills and problem-solving.
• CPR/First Aid certification and valid driver’s license.
• Warm, professional communication building trust with families and field staff.

Responsibilities

• Assess & Plan: Complete in-home RN assessments; craft individualized Care Plans with client/family approval.
• Coach & Supervise: Orient caregivers to each Care Plan, perform field competency checks, ensure ongoing compliance.
• Monitor & Document: Track vitals, condition changes, and interventions in HIPAA-secure EMR; elevate concerns promptly.
• Collaborate & Advocate: Coordinate with physicians, therapists, and schedulers to keep care seamless and client-centered.
• Quality & Compliance: Maintain audit-ready client files and uphold state home-care regulations.
