Job summary

Acacia Network in New York is seeking a Registered Nurse to evaluate consumer nursing needs for the IMT program and oversee physical health. You will provide referrals, perform blood draws, administer psychotropic meds, and coordinate care with a team of clinicians and outside providers.

The role requires documentation in NextGen EHR, after-hours availability, and a commitment to trauma-informed, recovery-oriented care. Bilingual Spanish/English is preferred.

Qualifications

POSITION OVERVIEW:

The Registered Nurse (RN) is accountable for evaluating consumer’s needs for nursing care on an ongoing basis. The RN is responsible for the physical health of Intensive Mobile Treatment (IMT) consumers. The functions below outline the responsibilities associated with this position. These functions are considered critical to effective department operations and Acacia’s mission. Satisfactory attendance and performance of functions in a competent, cooperative, and timely manner is required.

KEY ESSENTIAL:
  • Provide referrals as needed to primary and/or specialty medical care.
  • Conduct blood draws and administer psychotropic medication treatment, including administration of IV medications.
  • Prepare medication to be dispensed, and work closely with the Psychiatric Nurse Practitioner to dispense medication and monitor clients’ physical and mental wellbeing.
  • Conduct care coordination between other IMT staff members, pharmacy, health homes, and other outside providers
  • Record patient data in Promesa’s NextGen Electronic Health Record system.
  • Participate in grant related workshops and trainings.
  • Must be able to deliver in-person services after standard hours of operation as needed.
  • Perform clinical, compliance, and administrative duties as instructed.
  • Enhance patient’s health and self- management skills as developing wellness goals in the use of evidence-based practices and maintain record of these activities.
  • Participate in daily IMT staff and organizational meetings.
  • Practice according to the principles of recovery oriented and trauma informed service-delivery.
REQUIREMENTS:
  • Must hold a NYS-issued RN license
  • Previous relevant experience in a mental or substance use disorders required, experience with individuals experiencing homelessness a plus.
  • Ability to deal with sensitive issues with diplomacy and discretion and be able to multi-task with strong organizational skills.
  • Excellent interpersonal skills and able to communicate both verbally and in written form.
  • Ability to maintain a professional demeanor under pressure and operate with a keen sense of urgency and commitment to quality.
  • Bilingual (Spanish/English) preferred. Commitment to cultural diversity and sensitivity.
  • Excellent computer skills including Microsoft Windows, Excel, PowerPoint, and electronic communications tools: internet and email.
